Doctor Office Visit:  $5, $10, $15, $20
Prescription Drugs:  Varies by plan, but include 100 day supply for   
                             $5 and $10 dollar brand or generic copays; or 
                             $5 generic/$15 brand, $10 generic/$20 brand
Hospital:                 No Charge
Optical:                  (optional plan available $100 Allowance)
Chiropractic:           $10/30 visits
Infertility:               50% of Member Rate
